In designing the survey I made a few choices:

• Software specific vs. Generic
  

  I have attempted to make this survey industry-agnostic.

• Team vs. Organization

I realize that both the team-level agility and organization-level agility are important, so I left it ambiguous, and it is up to you to decide which level to refer to in your answers. Team, Organization, or both.

• Agile vs. General Org. Health

This survey is specific to agility in the organization/team, and excludes some important org-health factors.

Note that the answers are going to be subjective, and reflect an individual's view of what is "good/fast/right". This means that peers in the same team/organization are likely to provide different answers.
